Description
American Traveler is hiring an experienced RN Case Manager for an acute hospital setting, requiring 5+ years of RN case management experience and inpatient acute case management background.

Every other weekend required
Requirements
Active CA RN license required
BLS certification required
5+ years of RN Case Management experience required
Inpatient Acute Case Management (hospital) experience required
2 supervisor references obtained within the past year required
Additional Information
Responsible for care coordination and care transitions planning throughout the acute care patient experience
Collaborates with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely progression and transition of patients to appropriate levels of care
Conducts patient assessments to determine readmission risk and screens for ancillary supportive services including Palliative Care
Reviews medical records to ensure patients meet level of care requirements and documentation supports LOC determination
Monitors length of stay and collaborates with financial counselors on inpatient stay denials
Formulates transition plans in collaboration with patients, families, physicians, and community-based support services
Screens 30-day readmissions and creates effective transition plans with the interdisciplinary team
Identifies ED high utilizers and makes appropriate care plans and community resource referrals
Maintains current knowledge of CMS, Joint Commission Transitions of Care requirements, and Conditions of Participation
Candidates must reside within a 50-mile radius to qualify for local rate
Returning staff/travelers must have a 1-year separation before returning as a traveler
Maximum RTO is 7 days per contract, with only 1 holiday request approved per contract
Holiday coverage includes New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day
Must be available for lab work within 3-5 days of contract signing
