Job description
American Traveler is seeking an experienced RN Case Manager with an active AK license for an outpatient ambulatory care position requiring recent case management experience.
Requirements
Active, unrestricted Alaska RN license required at time of submittal
BLS certification required
Case Management certification preferred
Minimum of 1 year of case management experience in a clinic or hospital setting within the last 2 years required
Graduate of an accredited school of nursing
Bachelor's degree preferred
High school diploma or equivalent required
Responsibilities
Outpatient ambulatory clinic setting providing primary care case management services
Patient population includes individuals and families across the continuum of care
Monday through Friday schedule, 8:00 AM to 5:00 PM
13-week assignment with 40 guaranteed hours per week
Floating to other departments or assignments as needed
Documentation completed in the facility's electronic health record
Performs comprehensive patient assessments and develops individualized plans of care
Administers medications and treatments per provider orders and policy
Provides patient and family education related to care, medications, and discharge needs
Coordinates care with providers and interdisciplinary team members, including referrals and follow-up services
Advocates for patients and assists them in navigating the healthcare system
Delivers culturally responsive, patient-centered care
Exercises independent nursing judgment consistent with Alaska Board of Nursing scope of practice
Participates in quality and performance improvement activities and reports adverse events per policy
Provides guidance and delegation to ancillary staff and supports orientation and mentoring of staff and peers
Start date will be no less than 6 weeks after fingerprint cards are received by the facility
Housing and transportation are not provided by the facility
