Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 11/02/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours Employment Type: Travel Job Description & Requirements RN Case Manager StartDate: 11/2/2026 Pay Rate: $ - $ POSITION SUMMARY – RN Case Manager POSITION DUTIES – Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.
