Slate Healthcare is seeking a travel nurse RN Case Management for a travel nursing job in Oakland, California. Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 11/09/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Description In this role, the RN Case Manager will oversee care coordination and transitions for patients in acute settings. Collaborating with a multidisciplinary team, the nurse will ensure timely progression through care levels to optimize health outcomes and resource utilization. Daily responsibilities include communication with patients, families, and healthcare providers to facilitate effective care planning and prevent readmissions. This position is essential for improving patient experiences and outcomes within the healthcare continuum.
