Job description
St. Croix Hospice in Mason City, IA is seeking a compassionate Registered Nurse Case Manager to deliver 1:1, home-based, whole-person nursing care.
You will partner with an integrated care team to coordinate individualized care plans and support families through complex decisions with professionalism and empathy. The role requires an RN with an accredited nursing degree (associates, bachelors, or masters) and at least 1 year of acute care experience, hospice/home health preferred.
