Job description
American Traveler is hiring a Home Health RN in Mesa County, CO, requiring an active CO or compact RN license and offering a weekday schedule with on-call rotation.
Requirements
Active RN license in Colorado or active NLC compact license
Current BLS/BCLS certification
Valid driver's license, reliable transportation, and auto insurance required
Strong assessment and critical thinking skills
Comfort with documentation and EMR systems, HCHB experience preferred
Home health experience preferred but not required
Ability to work independently and manage a structured caseload
Responsibilities
In-person home health position providing skilled, patient-centered care in the community
Monday-Friday day schedule with a flexible day-to-day workflow
Includes participation in an on-call rotation
Manageable caseloads with one-on-one patient care
EMR documentation required, with HCHB experience preferred
Coordinates care with LPNs, therapists, caregivers, and providers
Perform comprehensive patient assessments and develop individualized plans of care
Provide skilled nursing care and appropriate clinical interventions
Monitor patient progress and communicate changes to the care team
Educate patients and families on medications, treatments, and care plans
Complete timely and accurate documentation in the EMR system
Work within a supportive, interdisciplinary team
Independent visits conducted in patients' homes
Ideal candidates are dependable, steady, adaptable, and proactive communicators
