Job description
American Traveler is hiring a Home Health RN Float requiring an active MA RN license and at least 1 year of certified home health and case management experience.
Requirements
Active MA RN license required, no pending or conditional licenses accepted
Minimum 1 year of experience specifically within a certified home health agency
Minimum 1 year of case management experience within a certified home health agency setting
AHA-issued certifications only
Flu vaccine required with no religious or medical exemptions allowed
Primary source verification of license required through both NURSYS and BON
Responsibilities
Home health setting covering the Lynn, Nahant, and Saugus areas within the North Region
13-week assignment with an 08:00-16:30 shift schedule
Weekend rotation required once every 6 weeks
Ability to travel to patient homes as assigned
Uses Epic for electronic charting and documentation
Responsible for assessing patient conditions, implementing care plans, and educating patients and families in the home setting
Performs admission processes and case management duties per organizational policies and standards
Requires thorough and timely electronic documentation and interdisciplinary communication
Care delivered in accordance with community health practice standards and organizational philosophy
Local candidates residing within 60 miles of Massachusetts General Hospital in Boston will be considered at a reduced rate
Uses UKG/Kronos for timekeeping
