Job description
Activate Care in Massachusetts seeks a full-time RN to travel to members' homes, conducting in-person health assessments for a managed care program. You will document findings in GuidingCare and hand off to care managers, supporting dually eligible members with dignity and culturally aware care.
This role requires independence, a MA RN license, and at least 1 year home health experience; you will travel up to 50% and must reside within a commutable distance of the MA territory.
