Job description
American Traveler is hiring an RN Hospice Case Manager for a home health setting requiring prior hospice experience and a VA or compact RN license.
Requirements
Active VA RN license in hand, or proof of compact license, required at time of consideration
Current BLS certification through the American Heart Association (AHA)
Prior hospice experience in a home health setting required
Specialty experience within the last year required
Responsibilities
Home health hospice case manager role
Day shift schedule, 5x8 hours (8:00 AM - 4:30 PM)
No on-call responsibilities
Floating may be requested based on facility needs
Serve as case manager coordinating hospice care for patients in the home health setting
Mileage reimbursed at the IRS rate
Local candidates accepted at the same pay rate
Prior permanent employees must have a 6-month gap in employment before returning as a traveler
Keep requested time off to a minimum, ideally no full calendar weeks off and no more than 7 total days during the assignment
All time-off requests must be submitted with initial application and confirmed prior to assignment start
