We are seeking a Travel Occupational Therapist for a 14-week Skilled Nursing Facility contract in Brooklyn, New York . This post-acute rehab opportunity offers the chance to work with adult and geriatric patients while helping them improve independence, safety, mobility, and overall functional ability.
This is a great fit for an OT who enjoys working in a collaborative rehab environment, building relationships with patients, and helping individuals regain function during their recovery.

Responsibilities
Perform comprehensive occupational therapy evaluations and reassessments.
Develop individualized treatment plans focused on improving ADLs and functional independence.
Treat orthopedic, neurological, post-surgical, and medically complex patients.
Provide ADL retraining, upper extremity rehabilitation, transfer training, cognitive interventions, and adaptive equipment education.
Collaborate with nursing staff, physicians, PT, SLP, and case management.
Educate patients and caregivers on safety strategies, home modifications, and discharge planning.
Complete timely documentation and progress reporting.
