HumanEdge Health is seeking an experienced Travel Occupational Therapist (OT) for a 13-week contract assignment in Port Orchard, Washington. This opportunity is in a skilled nursing facility and inpatient/outpatient rehab setting, offering a collaborative team environment focused on patient-centered care and functional independence.
Assignment Details
nLocation: Port Orchard, WA
nStart Date: ASAP
nDuration: 13 weeks
nSchedule: Monday–Friday
nHours: 40 hours/week (5x8s)
nSetting: Skilled Nursing Facility (SNF) & Rehab
Responsibilities
Evaluate and treat patients recovering from illness, injury, or surgery to restore independence in activities of daily living
Develop and implement individualized treatment plans to improve functional mobility, self-care, strength, and safety
Perform functional assessments and adaptive equipment training as needed
Collaborate with interdisciplinary teams including nursing, PT, SLP, and case management
Document patient progress, outcomes, and plan of care adjustments in accordance with facility policies and payer guidelines
Requirements
Active Washington Occupational Therapist license (or ability to obtain prior to start)
Minimum of one (1) year of OT experience; SNF or rehab setting preferred
Current BLS Certification (AHA)
Strong clinical judgment, communication, and patient engagement skills
