Ardor Health Solutions is seeking a Physical Therapist for a travel Skilled Nursing/Rehab contract in Wilmington, Massachusetts .
Work with an adult/geriatric rehabilitation population helping patients regain mobility, strength, balance and functional independence following hospitalization, surgery, illness or injury.
Responsibilities
PT evaluations and reassessments
Develop and progress individualized plans of care
Therapeutic exercise and strengthening
Gait, balance and transfer training
Functional mobility and fall prevention
Assistive device training
Patient and caregiver education
Collaborate with nursing, OT, SLP and the interdisciplinary rehab team
Support discharge planning and safe transitions
