Job description
Role:
Travel Physical Therapist in a Skilled Nursing Facility (SNF).
Responsibilities:
Provide rehabilitation services, including geriatric rehabilitation, fall risk assessment, and mobility training. Collaborate with interdisciplinary teams and document patient progress using electronic medical records.
Qualifications:
Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program. Active Illinois Physical Therapy license. BLS certification may be required. Experience in SNFs or long-term care preferred.
Skills:
Proficiency in geriatric rehabilitation, teamwork, documentation, and mobility training. Ability to adapt to a travel environment.
Key Duties:
Deliver therapy services, assess patient needs, and support patient mobility and recovery in a skilled nursing setting. Work 8-hour shifts, 40 hours per week.
