Job description
Longevity Health is seeking an Advanced Practice Practitioner (APP) – Market Floater to support nursing home–level of care members across markets. The APP will perform comprehensive assessments, develop individualized plans of care, attend care team meetings, and coordinate with PCPs, families, and care teams to align goals of care.
The role requires travel across state markets, collaboration with clinical leadership, and autonomous practice within regulatory oversight.
