Job description
Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in‑home health evaluations. Travel is required (up to 100%), with overnight stays possible.
Clinicians will visit members’ homes, review medical history, perform physical evaluations, and operate via an iPad-based workflow. Qualified candidates hold NP/PA licenses, board certification, and can obtain multi‑state licenses.
