Job description
Signify Health is hiring a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations across a defined coverage area. The role involves traveling up to 100% of the time, conducting 45-minute visits with medical history review, physical evaluation and optional tests, using an iPad workflow; you are not required to prescribe or order labs.
Active, unrestricted state licenses and board certification are required; multi-state licensing may be needed.
