Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for health plan members. You will travel up to 100% across defined coverage areas, perform medical history reviews and physical evaluations, and use an iPad-based workflow.
Must hold active, unrestricted licenses with board certification and be able to obtain multi-state licenses as required.
