Job description
Signify Health, part of CVS Health, is seeking a Full-Time Clinician (Nurse Practitioner) to conduct in-home health evaluations across a defined coverage region. The primary workplace is the patient’s home, with up to 100% travel in licensed states.
Visits last about 45 minutes and include medical history review and physical evaluation, with optional diagnostic tests. An iPad and clinical workflow are used; prescribing or altering treatment is not required.
