Job description
Longevity Health is seeking an Advanced Practice Practitioner (APP) – Market Floater to provide collaborative primary care to Longevity Health Plan members. The APP will deliver Model of Care activities, perform comprehensive assessments, and coordinate with primary care teams to align care with each member’s goals, across Texas markets.
The role requires active licensure as an NP or PA, board certification, and the ability to travel within Texas.
