Job description
Gurwin Healthcare System in Commack, NY is seeking an Occupational Therapist to provide fee-for-service home care across Suffolk and Nassau County. The role requires an active New York State OT license and at least one year of OT experience, with home health experience preferred.
You will visit patients in their homes, complete assessments, develop plans and goals, document visits, and coordinate with the care team. Laptop/tablet use is common, and travel to client homes is required.
