Job description
Fairview Health Services is seeking an RN Case Manager to support the Vascular Clinic in Minnesota. The role travels to vascular clinics in Maplewood, Woodbury, and Wyoming and to surgery centers in Minneapolis and Maple Grove, with work hours 7:30 a.m.–5:00 p.m.
This is a casual, on-site position. The case manager coordinates care for vascular, vein, and wound patients, triages nursing calls, and conducts education visits.
