Job description
Advocate Health in Charlotte, NC is seeking an experienced RN for telephone triage and care coordination within the Patient Access & Care Team. The role requires 5+ years of RN experience, BSN preferred, and active licensure; Monday–Friday day shifts with 36 hours per week are expected.
You will assess and triage diverse patients, document encounters, and collaborate with practices, physicians, and care managers to deliver timely clinical solutions.
