Job description
InnovAge is seeking a skilled Registered Nurse to provide comprehensive nursing care across clinic and in-home settings. This role partners with the interdisciplinary team to develop and execute individualized plans of care for frail and elderly participants.
Responsibilities include medication administration, care coordination, and education for participants and families. Travel to participant homes and InnovAge centers is required to deliver safe, high-quality care and ensure regulatory
