Requirements:
Board Eligible (within 5 years of residency) or Board Certified in Urology - REQUIRED
Active CO License (or willingness to obtain) - (note in "Additional Submission Details" field)- REQUIRED
BLS and ACLS - REQUIRED
Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
Visa Support: J1 or H1B candidates are eligible for visa sponsorship. (note in "Additional Submission Details" field)- REQUIRED
DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - REQUIRED no exemptions
DEA Controlled Substance License (required once hired)
