Utilization RN Case Manager – Discharge & Care Coordination

Job description

Kaiser Permanente is seeking a Case Manager Utilization RN to coordinate patient admissions and discharge planning in Moreno Valley, CA. You will collaborate with physicians and the multidisciplinary team to ensure appropriate care levels and safe discharge plans.

This role requires an active California RN license and experience in case management is preferred; the position is full-time on-site, with a focus on utilization management and continuous care coordination.
