Job description
Mass General Brigham in Somerville, MA is seeking a Clinical Pharmacist to optimize health outcomes within our Care Management teams. You will perform comprehensive medication management reviews, evaluate therapies for high‑risk, chronically ill patients, and support quality initiatives under ACO contracts.
This full‑time, hybrid role requires a PharmD, Massachusetts licensure, and BCPS/BCACP/BCGP certification is recommended.
