Job description
Upperline Health is seeking a Practice Success Partner to guide partner practices through the Value Based Care program enrollment, ensuring understanding of processes, documentation, and technology.
You will onboard clinics, analyze enrollment data, and drive performance improvements while coordinating with care teams to support patients. Travel up to 50% to partner practices; remote options available for eligible states.
