HarmonyCares seeks a Value-based Physician Leader to guide home-based primary and palliative care for medically complex, home-limited patients. In this role, you will lead an interdisciplinary team delivering in-home visits, telehealth, and coordinated care management. You will design value-based care plans, reduce avoidable hospitalizations, and drive quality metrics.
