Job description
Oak Street Health in Kalamazoo, MI is seeking a Nurse Practitioner to deliver effective, value-based primary care for adults on Medicare. The NP works with a collaborative care team to provide longitudinal care, conducts wellness visits, and coordinates with physicians and specialists to keep patients healthy and out of the hospital.
The role requires an MSN/NP credential, active NP and DEA licenses, and US work authorization.
