Job description
Oak Street Health is seeking a Primary Care Physician to provide equitable, value-based healthcare to local Medicare populations within our neighborhood primary care centers. You will diagnose and treat patients, work within a collaborative care team, and focus on outcomes that reduce hospital admissions and improve preventive care.
