Job description

HarmonyCares, a leading provider delivering in-home and community-based care, seeks a Value Based Care Physician Leader for Houston. You will guide a growing team to improve HEDIS and quality outcomes for high-risk Medicare patients under a mobile home-based care model.

You will supervise the clinical pod, ensure accurate diagnoses, documentation, patient safety, and collaborate closely with APPs and regional leadership to advance value-based care across our network.
