What You’ll Do
Deliver clinical excellence through comprehensive in-person evaluations.
Spearhead holistic care plans in collaboration with your dedicated IDT team.
Manage critical care transitions and long-term placement for seniors.
Champion patient needs within the Medicare/Medi-Cal framework.
