24 hour call to include 8 gratis hours.
Facility Location: 2222 N. Nevada Avenue, Colorado Springs, CO 80907
Setting: Call Trauma Level II

Required Procedures/Job Duties: Abdominal Open/Endovascular
EMR: EPIC
Support Staff: None
Reason for Coverage: Locums coverage for existing surgeons' time off.
Dealbreakers (Do Not Present): lack of availability for all dates
Submission Requirements **MUST NOTATE ALL DURING NAME CLEAR**
Board Certification Vascular Surgery - Required
Active CO license - Required or IMLC (LOQ in hand at time of submission)
Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
Must disclose # of shifts per month ongoing provider can offer to the team at submission - Required
Must be available for dates detailed in job description & disclose which shifts they can cover to be considered for ongoing dates - Required
Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required
Clean Malpractice/background - Highly Preferred - Case by case, include explanation
