Job description
Valor Healthcare, Inc. is seeking a Primary Care Pharmacist for a VA-aligned clinic in Glens Falls, NY. You will interpret orders, counsel patients on proper medication administration, and support Patient Aligned Care Team metrics.
The role requires PharmD, residency and board certification, ambulatory/primary care experience, and VA contracting familiarity. You will document in VISTA/CPRS, ensure safety and compliance, and deliver outstanding patient service to veterans.
