Job description
The Home Care Field RN at Carle Health serves as a critical link between patients, families and the interdisciplinary care team in central Illinois. Working independently and with RNs, LPNs, and therapists, you’ll coordinate skilled nursing care, education, and care plan implementation across multiple counties.
You will perform complex wound care and manage PICC lines, catheters, drains, and IV infusions, while ensuring compliance with Medicare home health benefits and documentation standards.
