Job description
Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and provide skilled nursing care across designated home health settings. You will work within an interdisciplinary team to manage patient plans, perform assessments, and support families in achieving optimal health outcomes on a county-wide service area.
You will oversee home visits, ensure regulatory compliance, and guide nursing staff and aides, while maintaining care continuity and effective communication with physicians
