Job description
Carle Health is seeking a Home Care Registered Nurse/Field Case Manager to provide skilled nursing visits and coordination across the home health continuum. The role supports patients and families with education and care planning in the field, working within an interdisciplinary team.
You will perform wound care, IV therapy, catheter care and other complex nursing tasks while ensuring regulatory compliance and high-quality patient outcomes. A sign-on bonus and mileage reimbursement are offered.
