Job description
Carle Health is seeking a Home Care Registered Nurse/Field Case Manager to coordinate and provide direct and indirect patient care across multiple counties. The role involves skilled nursing assessments, wound care, PICC line management, and collaboration with an interdisciplinary team to ensure optimal patient outcomes.
Qualifications include IL RN licensure, BLS certification, a valid driver’s license and auto insurance, with at least 1 year of nursing experience.
