Job description
Carle Health is hiring a Home Care RN/Field Case Manager to coordinate and deliver direct and indirect patient care in the home setting across specified counties. You will lead complex wound care, manage central lines, and partner with a multidisciplinary team to ensure high-quality outcomes.
Qualified candidates have an IL RN license, BLS, a nursing diploma, and at least 1 year of nursing experience. A driving license and auto insurance are required for field visits.
