Job description
Carle Health is seeking a Home Care RN/Field Case Manager to join our multidisciplinary team delivering skilled nursing care at patients' homes. You will coordinate care plans, educate families, and perform comprehensive nursing assessments across assigned counties.
This role emphasizes autonomy, ongoing documentation, wound care including VAC, and collaboration with LPNs, HHAs, PTs, MSW and physicians while adhering to regulatory standards and delivering high-quality patient care.
