Job description
Carle Health is seeking a Home Care RN/Field Case Manager to join our Home Services team. You will coordinate and provide skilled nursing care within the patient's home and lead an interdisciplinary team to meet clinical goals.
Responsibilities include wound care, PICC line management, and complex case management across Champaign, Vermilion, Edgar and surrounding counties, with opportunities for professional growth and supportive supervision.
