Job description
Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and provide skilled nursing care in patient homes across our service area. You will perform assessments, communicate with families, and collaborate with an interdisciplinary team to promote independence and wellness.
The role offers autonomy with 24/7 clinical backup, flexible scheduling and mileage reimbursement. Requires an IL RN license, nursing diploma, and relevant home health experience.
