Job description
Carle Health is seeking an experienced Home Care RN/Field Case Manager to join our home services team in central Illinois. You will coordinate and deliver skilled nursing care, educate patients and families, and collaborate with an interdisciplinary team to optimize patient outcomes.
The role includes performing wound care, managing PICC lines, and ensuring compliant documentation, with a focus on timely admissions, recertifications, and care planning. A valid IL RN license and BLS are required.
