Job description
Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and deliver skilled nursing care across the service area. You will work independently within an interdisciplinary team, performing wound care, IV administration, PICC line care and comprehensive case management.
You will ensure regulatory compliance, complete OASIS assessments, and collaborate with physicians, care coordinators, and family members to arrange services and support patient outcomes.
