Job description
Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and provide comprehensive nursing care in the home setting across multiple counties. You will educate patients and families, perform skilled nursing assessments, and support the interdisciplinary team to promote independence and wellness.
Responsibilities include complex wound care, central line management, and case coordination. A nursing degree, 1 year of nursing experience, and valid driver’s license are required.
