Job description
Carle Health is seeking a Home Care RN/Field Case Manager to join our interdisciplinary home health team in central Illinois. You will coordinate care plans, educate patients and families, and perform skilled nursing assessments and care across diverse patient populations.
Responsibilities include complex wound care, PICC line management, central line care, OASIS/485 changes, case management, and collaboration with physicians, home health aides, and other disciplines to ensure seamless care
