Job description
Carle Health in Illinois is seeking a Home Care RN/Field Case Manager to coordinate and provide skilled nursing care across multiple counties. You will work independently as part of an interdisciplinary home care team, performing assessments, planning care, and educating patients and families.
This role includes complex wound care, PICC/Hickman line care, IV infusions, central lines, and supervision of Home Health Aides.
