Job description
Carle Health is seeking a Home Care RN/Field Case Manager to join our interdisciplinary home care team in central Illinois. You will coordinate care plans, educate patients and families, and perform skilled nursing assessments and care in patients' homes.
Responsibilities include complex wound care, PICC line care, central line management, and collaboration with PT/OT/MSW, with emphasis on regulatory compliance and accurate documentation for admissions and ongoing care.
