Compensation: $40-$52 per hour + Mileage Reimbursement

What You'll Do
Complete routine and PRN hospice visits throughout your assigned territory.
Perform comprehensive nursing assessments and ongoing symptom management.
Provide pain management, wound care, medication education, and comfort-focused interventions.
Partner with physicians, social workers, chaplains, hospice aides, and families to coordinate individualized plans of care.
Maintain accurate, timely documentation within the EMR.
What You'll Bring
Active Minnesota Registered Nurse (RN) license.
Hospice experience preferred, with consideration given to nurses from home health, ICU, oncology, palliative care, or other end-of-life care settings .
Excellent assessment, critical thinking, and communication skills.
Confidence working independently in a field-based environment.
Reliable transportation and a valid driver's license.
