Weekend Nurse Liaison (RN)Looking for a full-time nursing career with a consistent weekend schedule? As a Full-Time Weekend Nurse Liaison, you'll work three 12-hour shifts each week (Friday-Sunday) while serving as a trusted clinical resource for patients, families, hospitals, and referral partners throughout our community. This role combines the clinical expertise of a hospice nurse, the relationship-building skills of a healthcare liaison, and the coordination responsibilities of a care transitions specialist, making it an ideal opportunity for experienced nurses seeking a highly autonomous and impactful position.This Is Not a Traditional Hospital Staff Nurse RoleWhile you will be primarily based within assigned hospitals, this role differs significantly from bedside nursing. Instead of managing a hospital patient assignment, you'll serve as a clinical liaison and hospice expert by:Rounding throughout assigned hospitalsMeeting with physicians, case managers, discharge planners, and hospital leadershipCoordinating hospice and palliative care referralsConducting informational visits with patients and familiesFacilitating goals-of-care conversationsSupporting active Transitions LifeCare patients during hospitalizationsCompleting hospice General Inpatient (GIP) admissionsTraveling between assigned hospitals and community locations as neededThis role is ideal for nurses who enjoy care coordination, relationship management, hospice admissions, and helping patients navigate complex healthcare decisions.What You'll DoCoordinate Referrals and Transitions of CareServe as the primary point of contact for referral coordination across all Transitions LifeCare service linesEvaluate referrals and assess eligibility for hospice, palliative care, and other organizational servicesFacilitate seamless transitions from hospital to home, facility, or hospice inpatient careCollaborate with discharge planners, physicians, social workers, and care teams to coordinate appropriate care plansSupport Patients and FamiliesMeet with patients and families to provide education and guidance regarding hospice, palliative care, and available support servicesConduct informational visits within hospitals and community-based settingsFacilitate goals-of-care conversations and support informed decision-makingProvide emotional support during difficult transitions and end-of-life discussionsServe as the Hospital Hospice ExpertRound regularly within assigned hospitalsBuild strong partnerships with hospital leadership and clinical teamsEducate healthcare professionals on hospice eligibility, levels of care, and Transitions LifeCare servicesProvide in-service training and educational presentationsManage Hospice General Inpatient (GIP) PatientsComplete General Inpatient hospice admissions within assigned hospitalsFunction as the hospice RN Case Manager for GIP patientsCoordinate the hospice plan of care with hospital staff and interdisciplinary team membersMonitor patient status and ensure appropriate symptom management and care deliveryBuild Referral RelationshipsPartner with Provider Relations colleagues to strengthen referral relationshipsAssist with account management, customer service initiatives, and educational outreachPromote awareness of hospice and palliative care services within assigned hospitalsHelp grow referral volume through exceptional service and responsivenessWho Thrives in This RoleStrong clinical decision-makersExcellent communicators and educatorsSkilled relationship buildersComfortable working independentlyPassionate about patient advocacyOrganized and highly adaptableExperienced in navigating complex healthcare systemsMotivated by improving patient and family experiencesRequired QualificationsCurrent Registered Nurse (RN) license in North CarolinaMinimum two years of recent clinical nursing experienceStrong assessment and critical thinking skillsExcellent communication, organizational, and interpersonal skillsAbility to independently prioritize workload and manage multiple responsibilitiesStrong computer proficiencyCurrent CPR certificationValid North Carolina driver's licenseReliable transportation and automobile insuranceAbility to travel throughout the service area and function effectively in hospital environmentsAbility to lift and/or carry up to 50 poundsPreferred QualificationsBSNHospice experienceHome health experiencePalliative care experienceHospital case management experienceSkilled nursing experienceKnowledge of Medicare hospice regulations and levels of careExperience with hospital discharge planningFamiliarity with EMR systems, Outlook, and Microsoft Office 365Why This Role MattersMany patients who could benefit from hospice or palliative care are first identified in the hospital.
